Consent to the Processing of Personal Data
I hereby, acting freely, of my own will and in my own interests, confirm that I have read the Personal Data Processing Policy of LLC «DNA Test Center» and give my consent to Limited Liability Company «DNA Test Center» (hereinafter referred to as the Data Controller) to process my personal data under the terms and conditions set out below.

2. Purposes of Personal Data Processing
I consent to the processing of my personal data solely for the following purposes:
- identifying me as a customer;
- concluding and performing the contract;
- placing an order for a genetic test;
- receiving and registering biological samples;
- conducting laboratory testing;
- interaction with the laboratory performing the genetic testing;
- issuing the test results;
- providing consultations;
- contacting me regarding the performance of the contract;
- processing my requests and inquiries;
- complying with the legislation of the Republic of Belarus.
3. Categories of Personal Data
Depending on the purpose of processing, the following personal data may be processed:
- last name (surname);
- first name;
- patronymic (where applicable);
- date of birth;
- gender;
- telephone number;
- email address;
- residential address (where necessary);
- information about the ordered service;
- contract details;
- payment information;
- information contained in requests and inquiries;
- test identifier;
- information relating to the biological sample;
- genetic test results;
- other information voluntarily provided by me.
4. Genetic Data
I acknowledge that the performance of a DNA test involves the processing of special categories of personal data, including genetic data.
I understand the purposes of such processing and consent to the processing of these data solely for carrying out the genetic test I have ordered and issuing the corresponding test results.
5. Processing Activities
I hereby consent to the following processing activities:
- collection;
- recording;
- systematization;
- accumulation;
- storage;
- updating (amendment);
- use;
- transfer;
- disclosure;
- anonymization;
- blocking;
- deletion;
- destruction.
Processing may be carried out both with and without the use of automated means.
6. Transfer of Personal Data to Third Parties
I have been informed that my personal data may be transferred to persons engaged by LLC «DNA Test Center» for the purpose of performing the contract and conducting the genetic test.
Such transfer shall be carried out only to the extent necessary to achieve the purposes of personal data processing.
7. Cross-Border Transfer of Personal Data
I acknowledge that, for the purpose of conducting the genetic test I have ordered, my personal data and the information required to identify my biological sample may be transferred to a partner laboratory located in the Russian Federation.
Such transfer shall be carried out solely for the purposes of conducting the genetic test and performing the contract.
I confirm that I have been informed of the cross-border transfer of my personal data and consent to such transfer to the extent necessary for the provision of the requested service.
8. Retention Period for Personal Data
Personal data shall be processed:
- for the duration of the contract;
- for the retention periods established by the legislation of the Republic of Belarus;
- or until the purposes of processing have been achieved, unless a longer retention period is required by law.
9. Rights of the Data Subject
I have been informed of the rights provided for by the Law of the Republic of Belarus No. 99-Z dated May 7, 2021, «On Personal Data Protection,» including the right to:
- receive information regarding the processing of personal data;
- obtain information about the disclosure of personal data to third parties;
- request amendments to personal data;
- request the cessation of personal data processing;
- request the deletion of personal data in cases provided for by law;
- withdraw this consent;
- appeal the actions of the Data Controller to the National Center for Personal Data Protection of the Republic of Belarus or seek judicial protection of my rights.
10. Withdrawal of Consent
I acknowledge that this consent may be withdrawn by submitting a written request or an electronic request to LLC «DNA Test Center.»
Withdrawal of consent does not affect the lawfulness of the processing of personal data carried out before such withdrawal was received.
11. Confirmation by the Data Subject
I confirm that I:
- have read the Personal Data Processing Policy of LLC «DNA Test Center»;
- have read the Cookie Policy;
- have received information about the purposes of personal data processing;
- have received information about the categories of personal data being processed;
- have been informed of the possibility of cross-border transfer of personal data;
- understand the consequences of granting or refusing to grant this consent;
- provide this consent voluntarily.
The date on which this consent is given shall be the date of submission of the relevant form on the website or the date of signing this document.
